Geriatric Medicine Consultations

Geriatric Medicine consultations provide medical recommendations to patients, families/caregivers, and their physicians regarding the evaluation and management of geriatric conditions.

Each consultation will focus on reviewing goals of care and helping patients to make decisions regarding conditions and syndromes that are especially common in older adults including:

  • Decline in function associated with multiple medical conditions
  • Decline in quality of life associated with:
    • Chronic pain
    • Depressed mood and/or anxiety
    • Sleep difficulty
    • Other symptoms
  • Use of multiple medications (polypharmacy)
  • Falls, fear of falling and gait abnormalities
  • Memory loss and dementia
  • Bladder problems such as overactive bladder and urinary incontinence
  • Possible need for additional in-home assistance or long term care
  • Decision making capacity and executing advance care directives such as living wills and durable power of attorney for
    health care
