Thank you, Florida Blue!

Florida Atlantic University’s Schmidt College of Medicine expresses its appreciation to Florida Blue for their grant to launch our wellness hub, in coordination with our FAU Medicine primary care practice.

The grant was awarded to support the initiation of FAU Medicine Wellness Hub, a community focused initiative promoting wellness and healthy aging. This initiative works in tandem with FAU Medicine Primary Care, a new patient-centered and evidence-based practice operated under the auspices of the Schmidt College of Medicine, a nationally recognized innovator in medical education.

 FAU Medicine Primary Care and the associated Wellness Hub:

  • address an existing gap in integrative, accessible primary care for the Boca Raton community;
  • lead innovation in primary care through a value-based model that improves the patient experience, enhances population health and wellness, and lowers health care costs; and
  • engage our community through a public health and wellness hub, providing resources and training related to mindfulness, nutrition, yoga, tai chi, and other health promotion activities and strategies.

The mission of Florida Blue is to help people and communities achieve better health outcomes. Through the program “Build Healthy, Strong Communities”, Florida Blue offers grants to support programs related to improving education/literacy in health and human services; arts and culture; and community development.

The objective of this specific grant is to enhance the FAU Medicine primary care practice by incorporating training, research, and quality improvement to serve the residents of Southern Palm Beach County. It also aims to engage the adult clinical population, and others from the community, in public health and wellness improvement initiatives.

Under this grant, the FAU Medicine Wellness Hub expects to conduct up to 120 public health and wellness hub activities annually, and serve an average of 200 participants per month, with the goal of 20% of participants reporting at least one lifestyle change intention.
